I would like to change my passport family name to my married family name, i worked in freeport Bahamas as care giver and married to a Bahamian. When i went home to Philippine last may for vacation i wanted to change my passport but i can't coz i don't have the patch number from the embassy here, so they advised me to change it here. so please can u help me how and what are the requirements? Is passport renewal now fully implemented in our phil. embassy in cuba? What are other documents needed to bring or to be submitted and is there a need to schedule or walk-in will do upon application. Thanks
im so thankful that we have now a Honorary Consul here in Nassau Bahamas.in the person of Dr.Gonzalo Bolisay who assist,help Filipinos who have/had problems.

Good morning,
Oliver thanks for taking my phone call yesterday.
Here is the position.
Charlotte DeLeon, a Filipino National worked in Bermuda for just over 2 years. December 2007 to January 2010
January 2010. - 1 week before she was to finish her contract, she was falsely accused of theft, by her employers.
Her employment was terminated.
Her employers have yet to produce anything substantial in the way of proof.
May 2010. - They pressed charges with the police, who after looking at their evidence, did not even contact charlotte, for an interview.
They filed the complaint with the police to stop a Labour Tribunal Hearing.
February 2011. - We are presently filing a claim in the courts of Bermuda for compensation, and deflamation of character
Charlottes Passport expired in May 2010. It was in the possession of the Department of Immigration here in Bermuda at the time.
Charlotte would like to go home on the 26th or 27th March. Mainly to attend her sister’s wedding.
She will have to come back to Bermuda for the court case, at some point in the future.
We need to get her passport stamped, or some sort of document for her to be able to travel home
And she can renew her passport when in the Philippines.
