Are you the same John who migrated to Canada ?
I had made a detailed post about this last may on this site
Head to your local MLA or corporator and explain your situation and ask for a letter from them to the BMC to make the name change
Visit your BMC and speak to the health officer or the person the corporator has a contact with and it will be done
It takes a month for a small correction in your parents name but I don't think they make any changes to the child name but you can try
Dont give any agent money but you can try giving a small amount to the staff if they ask
Has the cases reached October

How long does it take to get your assent once your birth is registered in Portugal ?
People anyone of yu applied for OCI with Revocation order from passport office..???
Cause FRRO office in panjim says they did not recieve any circular from MEA....is it real news or fake news on newspaper about OCI application ????
